Sometimes these can be tough because it’s a case of what you don’t see, ie the grey-white interface. Where don’t you see it? Anywhere. Add to that almost no sulci on either side and you have to be thinking of:

Answer: hypoxic-ischemic brain injury (ie global anoxia)

This is generally pretty obvious clinically (unless patient is intubated). MRI of course can show you a lot more detail. But the reference provides a new review of radiology signs of brain ischemic injury
